Bug Description
Due to an earlier change, NUnit allows non-public test fixtures when explicitly marked with [TestFixture]. We need to
1) Either allow non-public test fixtures all the time - including implicit test fixtures without the TestFixture attribute or define precisely when non-public fixtures are OK and when they are not.
2) Update the documentation to indicate how it works.
