konqueror: /usr/share/icons/hicolor/32x32/apps/konqueror.png

As we can see the icons are shipped along side the apps, so the missing icons in the menu are most likely not problems with the packages themselfs.

Bouncing bug back to gnome-panel, since I could only do wild guessing on what is causing the issue in particular (pixmap cache? icon theme not falling back to hicolor?)

not a gnome-panel bug in any case, either wrong .desktop or icon cache or user files or whatever but not a bug there
