Binary package hint: ubiquity
The issue happens on the current karmic rc image and seems to happen sometime when clicking on the wm close button while ubiquity is loading an another wizard tab. I got the issue the first time after a partitionning error, I clicked continue and trying to close ubiquity then to report the bug while the partionner tab was being reloaded. The issue seems to happen in cases when the "do you really want to close ubiquity" dialog is displayed before the installer tab is changed, in those case none of the quit, cancel or wm buttons are working

There have been many changes in Ubuntu since that time you reported the bug and your problem may have been fixed with some of the updates. It would help us a lot if you could test it on a currently supported Ubuntu version. When you test it and it is still an issue, kindly upload the updated logs by running only once:
apport-collect 457183
and any other logs that are relevant for this particular issue.
