document downloads showing docs with numbers?

Bug Description
We've seen this before but I can't find any old bug reports or emails that cover this. It's been witnessed twice now in the last week on the new live server (including just now).
Click on a document link on a published web page and what you get is a document (in word or text format) which just displays a number, e.g.
7244670091066548457
Sometimes successive attempts to download the file result in success (you see the document that you are meant to see), yet other times it stubbornly just loads up a file that contains the seemingly unrelated number instead!
It's odd because it loads up a word document with the number if the actual download should be a word file, or it displays a text file with the number in it if the actual link is a text file.
What's going on here?
