[f] remove cpan modules from our version control

This has our modifications:

 lib-mvhub/lib/CGI/FormBuilder.pm

We should look at removing the modifications and using the current cpan
library which no doubt has advanced in the past 4 years

Alternatively we should use a more cgi::application friendly validator:

 lib-mvhub/lib/CGI/Parse/AccessLogEntry.pm

Is probably where it is because it's installer didn't understnad cpan,
it isn't used except in:

 ./app-mvhub/bin/weblogslurp.pl

which isn't being used and should be re-written anyway.
