[Bug $bug_id] should be added only if it isn't in the subject line already

Bug Description
When someone replies to a bug notification, they usually generate a
subject such as: "Re: [Bug 42] ..." Now when we send out the
notification about the comment, we simply add another [Bug $id] prefix,
resulting in a subject like: "[Bug 42] Re: [Bug 42] ...".
We should add [Bug $bug_id] only if it isn't already in the subject,
just like mailman and other mailing list system do.

IMO all of Malone's email handling should be respecified by an expert in automatic email processing.
