Crash during installation of 9.04

Booted from Ubuntu Desktop 9.04 (32bit version)
1. Set graphics mode to 'Safe'
2. Selected option to install

During install
1. Selected time-zone, keyboard, etc.
2. Manual partition for / (ext3) and swap
3. Requested boot loader be installed on /dev/sda
4. Install process started with formatting partitions...then crash
