Dead mouse cursor when useing multiple monitors

Bug Description

I have a monitor (VGA with a DVI converter) and a TV (DVI to HDMI) connected to ubuntu.

When I move the mouse from the monitor to the TV (for instance) a dead version of the cursor will remain on screen, roughly either a scroll bar or half a scroll bar's distance from the edge of the screen, the mouse will be fully functional on the TV.

The same happens going from the TV to the monitor.

I have multiple monitors setup and I have this issue as well, it has not happened in any previous versions of Ubuntu. In addition to this problem, everything now opens up on the primary screen only no matter what.
