Don't make me change pages to sign this code of conduct

Bug Description

If I'm looking at a code of conduct, I shouldn't need to go to a separate page to sign it. The instructions and form for signing it should be directly underneath.

In other words, /codeofconduct/whatever/+sign should be merged into /codeofconduct/whatever.

This bug seems awfully similar to #30456 -- both are saying "there should be fewer" pages for codes of conduct. Is there reason not to dupe them?

Bug 34056 is about /codeofconduct vs. /people/me/+codesofconduct. This bug is about /codeofconduct/whatever vs. /codeofconduct/whatever/+sign. Neither of the pages mentioned in bug 34056 are involved in this bug, so they're not duplicates.

It would be nice for both bugs to be fixed simultaneously, but that applies to any two bugs.
