Please try to make this happen again, and then attach /var/log/syslog and /var/log/partman from the running installer (without rebooting) to this bug report. We'll need those files in order to see what went wrong for you.

I was experiencing a similar issue except the installer reverted to the location selection step. I was using the Kubuntu Alpha4 live CD. I also could not mount my extended partitions after exiting the installer. It told me the device does not exist. After rebooting the partitions could be read normally. I think this bug may be related to bug 324987 - "Kernel *loses* partitions at partitioning stage; installer fails".

Colin, this exactly happens to me on alpha-5, and blocks me from installing jaunty at all.

I attach the log of "ubiquity --debug" as found in /var/log/installer/debug, running the live system.
