Bug Description

A utility should be written to create the directory structure described in http://medsphere.org/proposals/3. The utility should also create the global directory file, empty database files, and a tied account user if it doesn't already exist. A SSH key should be generated for the tied account user and the key should be configured with an SSH forced command to run ^ZU.

A suggested name for the utility is "ovinstanceadd". It should require at least one argument - the name of the instance to create.
