Binary package hint: openoffice.org
"Always save a version on closing" is an option on 'File>Versions instead of File' feature on OOo writer.
It is supposed to save a new version on exiting a document.
But it does not work if you save the document just before exit. As a result last edition will not be considered a new version of document, so you loose it next time you edit and save again that document.

I can confirm this bug with latest OpenOffice.org from repository in Jaunty.
