Convert review-submit to use LP merge proposals

Bug Description
Now that LP supports merge proposals, the review request submitter should use them instead of email and wiki.
Note that this blocks on there being an API for merge proposals.

~rockstar is telling me that the current plan is to deprecate this plugin and extend bzr send to do everything we need, so maybe this is not such a great idea.
