make faq files valid and well-formed

Bug Description

As with user and quick guides, this patch makes the faq files valid and well
formed. The following Changlog applies to all files:
[*] correct encoding on xml declaration from utf8 to UTF-8
[+] DocType Decl
[+] define xi:fallback
[*] position and define unbuntu image in valid position
[-] remove ubuntu icon from chapters, not required, invalid
[*] fix whitespace management to avoid unexpected character data errors in
validation
[*] replace quotes "" and '' with entities " " and ' '
respectively
[*] make itemized lists valid by wrapping value of listitems in para
[-] IconUbuntu.png from FAQ, Documents now use IconUbuntu.png found in
the root of trunk. No need for two instances of this object.
[*] comment out invalid links not in FAQ
[-] ubuntu.xsl, we already have the file in root of trunk.
[-] Makefile, we already have this file in root.
[-] Unofficial Ubuntu Starter Guide (v4.10)). No presentational formats in tree.
