+ buttons not functioning on edition edit page

The + buttons are not working on the edit page for the edition:

So any book's edit page there are those + buttons, inactive at the moment.

Yes, this is broken functionality more than likely from some broken
javascript. If you open error console on your computer, and clicking on
the + icon, get: "add_row is not defined"

Overall, this edit page looks pretty f-ugly too and breaks on diff.
sized browser windows.

For documentation purposes: Somehow the <script>...</script> section of /edition/edit was gone. Copied over from OL to fix.
