list moderation cancel URLs do not work

Bug Description
I sent a msg with a large attachment to a private mailing list, and the msg was (appropriately) held for moderation. I received a msg with the standard "Your email has been held for moderation" text, with a URL to click on to cancel the request.
That URL doesn't doesn't work (404 error).
Additionally, I think (but cannot confirm by myself) that the admin of the list doesn't receive a notification about the need for moderation.

We need to tweak the moderation template so that the cancel url isn't included. We don't support cancellation through Launchpad currently.
The admin should be receiving notifications of moderation. I get them for mailing lists on teams I'm an admin for. (In particular, I get one for your team Jane :)
