subscription options at subscribe time

Enable the subscriber to select subscription options
when signing up for a list. For instance, if someone
is already subscribed to a list from one email address
(which might be an alias that goes to a team), but
wants to add a second address that would be allowed to
post to this list (e.g. the address of a team member),
they want to subscribe the second address with the "no
delivery" option enabled from the start. Presently
they need to subscribe, then confirm, then go back and
set "no delivery". This should be able to be done at
subscription time via both the website signup page and
via the email interface.
