Moderator pending items not closed

When a moderator/admin receives email notification of
pending items and handles them, references in
$MAILMAN/lists/<list>/request.db are not correctly
cleaned up - so they get another notification the next
day of the same pending item.

Example: someone tried to subscribe to a non-open list,
and after selecting "accept" and submitting it, nothing
happened. This example is a bit more extreme, as the
command did nothing at all.

Example: a message sent to a moderated list has been
sent just fine, but the moderation request is still
sitting in the request.db more than a week later, after
having been accepted more than a dozen times. There
are no errors in any of the logs.

There were some bugs of this nature in earlier alphas, but I
believe they are all fixed in MM2.1beta1.
