Binary package hint: openoffice.org
When building a new presentation template, the slide design used by the first slide automatically gets renamed to that of the template filename, even if other master slides already have that name.
Steps to create:
1) File->New-
2) File->SaveAs->ODF Presentation Template (.otp)
3) * Create master views A and B, in addition to existing "Default".
4) Format->Slide Design->Select a Slide Design->A
5) File->Save
As the file is saved, master view A gets renamed to the template document's filename.

I believe I have found the related upstream bug, but it is a much larger issue than just the slide getting renamed.
