io-error and raise/strerror should include &error

Bug Description
... so they always raise &serious conditions, so if the base exception handler gets the condition, it does not return. (I'd say make the &error the first condition.) (I didn't look for any other spots that should but aren't raising a &serious.)

&i/o inherits from &error so it is already an &error, similar, &error is already &serious.
