My firefox startup page opens multiple tabs to various site that requires authentication and therefore needs access to the security manager.
Using firefox 2 (on gutsy), the security manager password request is brought once the first time access is needed and once my password is validated all the sites gets accessed fine.
Using firefox 3 (on hardy), the security manager password request is brought up as many times as I have tabs that requires access to the security manager (the windows pile up), and I need to enter my password in each of them or else I get a 404 access error on the site.
This problem cannot be reproduced on firefox 3 beta 4 on a macintosh.

Could you tell me if you are using any plugins or extensions on Firefox? Also, what build of Firefox are you using?
You could also try to run Firefox via the terminal and see if you receive any output messages from it when the error occurs. If you do please attach them to this bug.
This information will help diagnose the problem.
