The installation process its says that the disk crashed due to maybe disk damage.

You should take a look at the end of the file UbiquitySyslog.txt which was automatically added to this bug report. This should give you some pointers as to where the problem lies.

However, Ubuntu 16.04 (xenial) reached end-of-standard-support on April 29, 2021 which was almost three years ago.

Please only install supported releases of Ubuntu. See this document for currently supported Ubuntu releases: https://wiki.ubuntu.com/Releases

I'm closing this as 'Invalid' as you don't appear to have found a bug in a supported release of Ubuntu.
