Bug Description

Steps to recreate:
* Download WAVE or aXe devtools browser extension.
* In the staff client, go to Circulation > Scan Item as Missing Pieces
* Scan the page using your browser extension of choice
* Note that the input is not associated with its label.

Note that we deviate slightly from Bootstrap guidance on labels in input groups: you can use <label class="input-group-text">.
