If a patron has a legal middle name and preferred name (either first or last) saved to their profile, the legal middle name persists to show as displayed. You can't use an empty string either as a workaround to get the legal middle name to not display.
Example:
Legal name: Sarah Anna Smith
Preferred name: Sam Smith
Displays as: Sam Anna Smith

If patron has a preferred middle name, the name display correctly:
Legal Name: Janet Anne Wise
Entered as Preferred First and Middle Names: John Andrew
Preferred name displays as John Andrew Wise
If patron has no preferred middle name, the primary middle name will display
Legal Name: Janet Anne Wise
Entered as Preferred First Preferred Name: John
Preferred Name displays as: John Anne Wise
It's expected behavior that you only need to add the preferred name elements that differ from the same primary name elements, but it looks like there's no way to create a preferred name that intentionally doesn't use the primary middle name or a preferred one.
In other words, there is no way for Sam Smith to just be Sam Smith.
This is true of the other name elements as well -- if the primary name has a suffix like Jr., there's no way for the preferred name to display with no suffix, and it's likely that most people whose primary name is a Jr. (or III, etc.) should not have those prefixes with their preferred name.
