Name link in help wiki header adds /UserPreferences to current URL

Bug Description

When logged into the help wiki, the name at the right of the header links to the current URL minus one level plus /UserPreferences.

For example: If you're looking at https://help.launchpad.net/DocTeam/UserGuide the name links to https://help.launchpad.net/DocTeam/UserPreferences rather than https://help.launchpad.net/UserPreferences

This is the same root cause as bug 197909.

We deployed my updated branch today which fixes this... however I missed the form controls for the logout button. I've pushed up a new revision and have a request in to deploy it. Won't be done until Monday though.
