Item alert types not displaying with alert

Behavior seen when testing 3.8 and 3.9
Browser used: Chrome Version 102.0.5005.115 (Official Build) (64-bit)

What I did:
In the Holdings Editor, I clicked on Item Alerts.
The alert type dropdown box displayed "New Alert Type."
I selected an Alert Type (e.g. Normal checkin).
I entered the text of the alert note.
I clicked on Add New.
I clicked on Apply All & Save in the Holdings Editor.
I click on Item Alerts to review my work.

What I expected:
To see the item alert type I had selected to display alongside of the alert note I entered.

What happened:
The text of my alert note was visible, but the Alert Type box did not display the alert type I had chosen. Instead, the dropdown box said "Alert Type."
I looked at the item in Holdings view/Action/Edit Item Alerts, and saw the same non-display of the alert type.

A patch is available in the pull request branch for bug 1956986.

I tested this code on Terran's test server (EG Head version). I followed the steps that Mary had listed and got the same result. After adding the item alert, applying it and saving the item, I selected Item Alerts again. My alert message displayed, but the alert type did not. Please see attached screenshot.

I tested and am seeing the item alert type with both new and existing item alert

Not sure what needs to happen next with this bug report but I can confirm that the fix committed to 3.9.1 for https://bugs.launchpad.net/evergreen/+bug/1956986 fixes this issue.
