Mouse receives power via USB when plugged in, immediately loses it. Mouse on other devices, USB port provides power to other mice.

Your kernel log is indeed full of USB errors. This is most likely a hardware fault in the mouse, the port or the motherboard. Otherwise it would be a kernel bug.
