Change Operator Credentials Retained After Cancel in Angular

Bug Description
Evergreen 3.7.0 and 3.8.0
On an angular page like Search the Catalogue if you go to menu -> Change Operator and enter in a username and/or password and then click Cancel the credentials you entered are still filled in if you click Change Operator again. This persists until you close the tab or go to an angularjs or dojo interface.
To reproduce:
1. Go to Cataloguing -> Search the Catalogue
2. Go to menu -> Change Operator
3. Enter a username and/or password
4. Click Cancel
5. Do a search and go into a record on that tab.
6. Go to menu -> Change Operator
7. The username and/or password is still filled in.
8. Click Cancel
9. Go to Cataloguing -> MARC Batch Import/Export
10. Go to menu -> Change Operator
11. The username and/or password is still filled in.
The username/password should be cleared when Cancel is selected.
