If an existing VM Cluster returns a different name on refresh, duplicate cluster and hosts are created

Bug Description

If a cluster is created, and a peer may return a different name for the cluster, this results in a new cluster being created, and all but the peer being refreshed are duplicated. This is due to a `VMCluster.objects.get_or_create` receiving the different name from the refreshing peer.
