Bug Description
The Bootstrap OPAC's login modal has a couple usability issues:
- When you open it, focus is not set anywhere. Focus ought to be given to the card number field.
- If you're starting from the barcode input, the tab order is "Request a Card", the help popover for the PIN field, and _then_ the PIN/password field. Tabbing from the barcode input should take you straight to the PIN/password input.
