Fails to load firmware for ipw2200 card

Bug Description

I have had my wireless card working perfectly for the last couple of weeks (this
is in breezy), and suddenly today, it has started to fail to load the firmware:

lspci of the card:

I have the lots of firmware files in /lib/hotplug/firmware, and oddly I can't
see anything that I have upgraded between the last boot when it was working and
now where it isn't working (it is still failing even after a couple of reboots).

(This is with linux-image-2.6.12-3-686 version 3.3)
