Angular: Should not show text in entry fields

In the new Angular Providers work, in the New Provider modal as well as on the details page for the Provider, the blank entry fields have light gray text as sort of a hint for what goes in the field.
However, I'd prefer those fields just be empty. Since the fields are right next to the bolded field name, I don't think they need the hint suggestion. And having the hint suggestion makes me think that there's something there already in that field when I'm looking at the provider info.
There's also field hints on the actual search form, and I think those should stay. Since there's no other labels for each of those fields, I think that's appropriate to have.

I've updated the title of the bug to better display that this is across all the Angular interfaces. There's some pertinent discussion about this in IRC: http:// irc.evergreen- ils.org/ evergreen/ 2020-12- 14#i_468158
I'd like to see the default as that they are *not* visible, and then per modal they could be enabled if it's appropriate for the given information, i.e. there's no other text elsewhere to indicate what that field should include.
