Bootstrap opac: Suspend hold at time of placement not working

Evergreen 3.6.0

When placing a hold, a patron can select to suspend the hold at time of placement by checking the Suspend box, the patron can also select an Activation date from the date picker.

Neither option is saved in the hold, resulting holds are active.

Note that the date picker should not allow choice of a date that is in the past. Bug 985957 fixed the past activation date issue for the TPAC.
