Bootstrap OPAC does not allow patrons to see/update per hold notification preferences

Evergreen 3.6

One of the components of bug 1570072 was to allow patrons the ability to see and update their per hold notification preferences while viewing their holds in the opac.

This functionality didn't make it into the Bootstrap OPAC.

I'm attaching a couple of screenshots of how this looks in the TPAC.
