SMS Carrier UI Allows for Duplicate Carrier Names

The SMS Carrier user interface (Administration -> Server Administration -> SMS Carriers) allows for duplicate names. This is particularly problematic for the patron registration Default SMS Carrier drop-down menu. Since the patron registration Default SMS Carrier drop-down menu currently does not include the region in parenthesis (like the hold messaging preferences drop-down does), this leads to duplicated options within the patron registration drop-down without further context. Even if region was currently displayed in the patron registration drop-down, it's still problematic to allow for duplicated names within the SMS Carrier field.
