Effects disabled when "pin dialog" is checked

Bug Description

in pre0.40
the "pin dialog" check box in extension dialogs turns "OK" into "Execute", but the button has no effect at all.

I think either the button should effectively run the extension, or the "Ok" button should simply be disabled (I suggest this last behavior!).

Confirmed in current SVN rev 17209 (jan 29) under Linux (fedora core 6) with 'grid' and 'spirograph' effects. This is a recent regression: 'execute' used to work...

The 'close' button also has no effect when the dialog is pinned: you must use close box in the titlebar.

Note that if 'live preview' is toggled before choosing 'pin dialog', then the checkbox remains checked, but live preview is turned off....
