installer crashed while installing on my dell insperion 3521

On dell 3521 inspiron series ubiquity never crashed till ubuntu 18.04 but this time it crashed saying inpput output error, am using USB live disk to install which i used to install on my friend's dell 5577 few hour ago. Am trying again now if this time it will work i will update this bug here...

This happened again trying with the same disk, am trying to check disk and remake a new live disk.
