authentication window got stuck over other windows at top left

Bug Description

The "Authentication required" screen appeared while I was changing software repository settings, but it is stuck there:

1. I can click "cancel" but there is no effect
2. The window is now always on top of any other window, despite the original Software Repository Settings windows has been closed (actually, terminated manually)

As you can see in the screenshot, The Firefox windows has focus now as I am typing in the address bar, but still the "dead" authentication window is on top.

I also faced this issue multiple times after upgrading to 18.10. Screenshot is attached

Behavior:
1) Authentication Required dialog stucks at top left of the screen
2) Password text field becomes disabled so we cannot enter anything
3) Cancel button can be hovered and clicked from the button left but it does nothing
4) Authenticate button doesn't show any hover effect

In my case, the buttons are completely inactive. I have a recording of my desktop, to see it reacting. I can use whtaever is behind, this is only on screen, it is not a real window, but it covers what is bellow.

Just tried restarting gnome shell (alt+f2, r) and it dissapeared.

ALT+F2, then type 'r' & hit enter.
