New Max Fine Rule fields need to be put back in more user-friendly order

Bug Description

Evergreen version 3.3

This bug is similar to bug https://bugs.launchpad.net/evergreen/+bug/1839874 and https://bugs.launchpad.net/evergreen/+bug/1839875

Administration -> Server Administration -> Circulation Max Fine Rules -> click New Max Fine Rule button

In previous versions of Evergreen, when creating a new max fine rule, the fields were displayed as Rule Name, Max Fine Amount, Is Percent.

In 3.3 the order of the fields is: Max Fine Amount, Rule ID, Rule Name, Is Percent.

The order in 3.3 isn't entirely user-friendly. The Max Fine Amount and Is Percent fields should remain ordered together.

Confirmed this is still an issue in 3.9 and the bug it depends on was resolved with 3.4
