wishlist: populate Default Phone notification from one of the other phone fields

This came up in user testing for the Update Holds Notifications project (bug 1570072).

The desire is for a prompt to ask the staff user about auto-filling in the Default Phone Number notification field from one of the existing phone number fields (Day Phone, Evening Phone, Other Phone).

My preference would be for the default phone setting to indicate the type of phone in the User Settings, and for the chosen field from actor.usr to be used to populate hold notification settings, rather than having the phone number copied into this field.
