Disable Place Hold button on submit

On the Place Hold form, the Submit button is not disabled after you click it. As a result, it is possible for users to inadvertently place multiple holds for the same title by clicking Submit multiple times before the result page is loaded. We should disable the button after it's clicked, just as we do for the search button.

+1 We've had a lot of issues with duplicate holds over the past few months that seem to be a problem with double-clicking.

Working branch user/jeffdavis/lp1824410-disable-place-hold-button-on-submit replaces the Place Hold button with a spinner on click, just like the search button:

This looks like a duplicate of bug 1029601 (which has a pullrequest) to me. Does anyone disagree before I mark it as such?

I *knew* this bug seemed familiar but my search didn't turn up the other one. Agreed that it's a duplicate.
