The migration code is meat for an upgrade to Pike. It can be removed on master as we've already passed Queens.

Remove v2 -> ml2 migration code
It's meant for an upgrade to Pike and we're now past Queens.
This commit doesn't remove the corresponding documentation.
Keeping old upgrade documentation available on master can be
convenient for a user planning an upgrade from older releases.
