On amd64 this looks to be a race between creating disk partitions and having usable devnodes created by udev (wait, check?):

The partition table has been altered.
Calling ioctl() to re-read partition table.
Syncing disks.
The file /dev/loop12p1 does not exist and no size was specified.

F: Failed to mkfs -t ext3

On s390x the test case still(!) does not handle the fact that there is no grub used:
