pt-query-digest: 95th < min

Bug Description
I ran pt-query-digest on a slow log with default parameters. Stats for one of the queries:

For Rows examined, you can see that 95% (2.26M) > min (2.37M). It also looks unlikely that median and 95% are the same - but of course it's possible.
I cannot share the slow log, but please let me know if I can provide more non reserved info.
