No default "grade" action

Bug Description

Right now, on a question, "Show answer" is the default action (i.e. you can select it with Space or Enter). That's good.

Then grades can be attributed from keyboard by hitting 0 to 5. This is good too.
But one of the grade button (probably the last selected from previous card) is also selected by default, so hitting Space/Enter again would grade your card, likely not to the grade you wanted.

Steps:
- Be in the step when you are asked to grade your answer.
- Hit Space.

Result: you go to the next card and guess that "some grade" has been attributed to the previous one, but have no idea which.
Expected result: nothing should happen. In this step, you are expected to grade explicitly, either by hitting 0 to 5 buttons with the mouse or hitting the keyboard key 0 to 5.

I would like to propose that none of the grade buttons are selected by default so that hitting Enter or Space by mistake has no consequence.

This is not a bug, but a feature... The default grade is different for unlearned and cards to be revised, though.

Are you telling me there is actually a documented logics? What happens when I hit Space on the grading step? What annoys me in particular is that sometimes I would hit Space by mistake and I just have no idea what was graded for the previous card, nor can I come back to edit the grade.

What are these default grades?

You should be able to see visually what these default are because of a thicker border. The default for 'old' cards is 4, and from memory, I think it's 2 for new/failed cards.
