EFI crashed while trying to reinstall xubuntu in existing LVM and changing root from EXT4 to BTRFS

The installer crashed with the included error logs while trying to install Xubuntun 16.04 2 on an existing LVM and overwriting the / ext4 partition with a btrfs partition, and the existing SWAP/EFI/boot partitions.
