Bug Description
NewDeadManager is documented as returning a manager that returns an error
on all its operations, but actually the Claim and Token methods will panic
if called. This panic has been seen here http://
To fix, perhaps initialize the Manager with a dumb Secretary instance that returns
an error from all operations.
