Z system installation of 17.04 has duplicate swap file size dialogues

Bug Description

When installing a Z System with 17.04 (Zesty) two dialogues related to swap file size are presented. The second seems redundant and although it asks for a maximum percentage of free space to be entered, there is no input field. Please see the attached screen shots.

Again, none of these are shown by default, you are not using the default priority for the installer?

Please note, installing with lower than default priority is not recommended for any users.

Please always attach the installer logs - either
a) file bug reports using ubuntu-bug from the installed system
b) attach /var/log/installer/* from the installed system
c) attach /var/log/syslog and /var/log/partman from the d-i environment

(you may want to use included shell to extract those, and e.g. $ anna-install openssh-client-udeb, might be useful to scp files from the d-i environment)

The two questions are valid - one is asking for absolute size maximum, the other is asking for relative (percentage) maximum.

I will check why the otherone is not showing up properly.

I did change debconf to medium, due to the system being on a vlan and bug 1635024.
