[2.2, UI] Image OS shows up twice in dropdown when deploying

Bug Description
In 2.2, when deploying via the web UI, the 'Image' dropdown contains duplicate (2) entries of 'Ubuntu'. This happened when I started with the 16.04 image and then added a second or third (tried 14.04 and 12.04) image OS. When I went back to just 16.04 the duplicate entry vanished.
This may be related to bug 1431156.
