Docs: Phonelist.pm has some broken formatting

The documentation for Phonelist.pm has a few small AsciiDoc formatting errors, which can been seen here:

The issues include:

- A missing backtick at the end of the first example URL (after "A couple of examples follow:")

- Multiple cases of incorrect quotation marks (e.g. ``user''), which should be replaced with single backticks (e.g. `user`)

NOTE: The Evergreen community generally does not use Github for managing pull requests, but documentation is the exception to that rule. In addition, docs changes are welcomed in whatever format is easiest for you, though AsciiDoc is preferred if you are able to provide it.
